Bacteriostatic water, explained

What benzyl alcohol does, why bacteriostatic water exists at all, and the single rule about punctures that most community advice skips.

The definition in one line

Bacteriostatic water is sterile water containing 0.9% benzyl alcohol as a preservative. That is the whole formula — the same preservative concentration in the officially labelled bacteriostatic diluents (0.9% benzyl alcohol with 0.9% sodium chloride, per the Hospira label). The name describes what the preservative does — it does not kill bacteria outright (that would be bactericidal); it inhibits their growth (bacteriostatic).

Why it exists

A sealed vial of sterile water is sterile until you put a needle through the stopper. After that, every puncture is an opportunity for contamination, and the liquid inside has nothing in it to slow bacteria down. Benzyl alcohol changes that: at 0.9%, it suppresses growth of most bacteria introduced by accidental contamination, which is why multi-dose diluent vials — and multi-dose drug vials generally — use it.

The practical consequence for research vials: a bottle of bacteriostatic diluent, once opened, remains usable for a stated period — the labelled products specify an in-use period after first puncture (the widely cited 28-day convention comes from multi-dose vial guidance) — while a bottle of preservative-free sterile water is effectively single-use once punctured.

What benzyl alcohol does not do

  1. It does not sterilize a contaminated vial — it slows growth; it does not undo contamination.
  2. It does not preserve the reconstituted peptide solution's chemical stability. The peptide's degradation chemistry does not care that the water is preserved.
  3. It is not harmless to every organism at every dose — benzyl alcohol toxicity is a documented paediatric concern in drug-safety literature, which is why some products explicitly exclude bacteriostatic diluents.

When preservative-free sterile water is the right call

Single-use contexts — where the whole vial is consumed at once — do not need a preservative, and some peptides or applications specify against one. Manufacturer documentation wins every time: where a supplier or label states a diluent, that statement reflects measured solubility and stability work on that exact product.
